HFEA: The Human Fertilisation & Embryology Authority
The Bath Fertility (BFC) is licensed and inspected by the Human Fertilisation & Embryology Authority (HFEA) and must abide by the HFEA Code of Practice as directed under the HF&E Act of Parliament (1990). All patients and their partners must be registered with the HFEA together with details of treatment cycles and their outcomes.
Under the terms of our licence, before offering treatment, we are required to consider the welfare of any resulting child as well as of any existing child who may be affected by the birth. You will be given HFEA Welfare of the child patient history forms in your information pack and you will need to complete them (one for each partner) and bring them with you to your consultation. It is your responsibility to disclose any relevant information.

Any information that BFC holds is confidential. Limited information has to be disclosed to the HFEA. We may only disclose information to other specified persons (for example your family doctor) if authorised to do so by you on the HFEA Consent to Disclosure (CD) forms. The only exception to this is where disclosure may be thought necessary to avert imminent danger to the health of the patient
